Access Card, Key and Account

For access card, room key, computer account and kth.se-account (no need to contact kortexpeditionen if no picture on card needed?).
Mail in advance so they can prepare everything.
They need the following information
- name
- current mailaddress?
- personnummer (or at least birth-date)
Access Card with name/picture - Kortexpeditionen
kortexp@admin.kth.se
They need the following information:
- name
- rights (behörigheter), e.g. "vanligt akademisk personal: NADA/CSC-anställd 43:03"
- period of valididty (giltighet till)
Confirmation from Mads or Viggo (via e-mail in cc is fine).
Computer, room equipment
- talk to Guillermo for nettop, screen, keyboard, network cable
